It is important to complete the "Request for Special Dietary Accommodations" form to indicate the need for special dietary restrictions for your student. This form is used to communicate specific allergies and medically necessary food substitutions with the School Nurse and Food Services Department. The district is required to have the "Request for Special Dietary Accommodations" form on file, completed and signed by a recognized medical authority in order to make any substitutions to your students' meals.
Your students' safety is very important to us. Without this form, your student would be served the standard meal options provided for all students during lunch and breakfast meals. Due to the federal nutrient requirements for meals offered to students, we appreciate your understanding that Nutrition Services is unable to make food substitutions based on individual preferences including religious or philosophical practices. Monitoring and enforcing dietary restrictions associated with religious or philosophical practices is the responsibility of the parent/guardian.
Have your health care provider complete the "Request for Special Dietary Accommodations" form and return it to school as soon as possible, so that may begin making arrangements for substitutions. The district will be available to meet with the household to discuss how the district can/will make the accommodations if the parent has concerns to ensure each student receives a nutritionally equivalent meal. If you have questions or concerns, please do not hesitate to contact Food Services.
